Physical activity is a significant aspect of promoting health and preventing diseases. It is one of the modifiable risk factors in managing chronic diseases such as obesity, diabetes, hypercholesterolemia, and hypertension. The Centers for Disease Control and Prevention recommends older adults to do at least two and a half hours of moderate-intensity aerobic activity and at least two or more days of muscle-strengthening activities every week. Routine engagement in physical activity is a significant part of healthy aging. The aims of this scholarly project were to implement a physical activity assessment tool and identify its effects on the rate of physical activity promotion among older adults in the primary care setting.
