High blood pressure, or hypertension, is an increasing issue among adults in the United States. With about half the adult population in the United States having hypertension, the risks of related events and fatalities continue to increase. Patients with high blood pressure only during the office visit, also called white coat hypertension, are often diagnosed and treated without considering their blood pressure readings outside the office setting. Masked hypertension or high blood pressure outside the clinical setting is also overseen. The lack of treatment can lead to chronic complications and death. Inadequate diagnosing and treatment have contributed to the increase of uncontrolled hypertensive adults. The lack of knowledge of evidence-based tools and resources can drastically affect patient care and increase healthcare costs. This evidence-based project aims to advance the knowledge of healthcare providers and patients by implementing a self-measured blood pressure program to help patients improve their blood pressure (BP) readings, self-management behaviors, and medication adherence.
